Despite the adult certification, the Central Board of Film Certification (CBFC) mandated several specific dialogue changes to the film's final cut. The Examining Committee notably asked for the word Item to be replaced with Kumari across the film's narrative. Furthermore, the board ordered the muting of one specific word and the complete deletion of another deemed inappropriate. While four other words were also replaced with more suitable terms, specific details regarding those dialogues were not disclosed in the final cut list.
While the film's romantic and intimate sequences passed through the board without a single modification, the action entertainer aspect of the movie faced strict scrutiny for its portrayal of raw violence. To maintain the A rating while adhering to safety guidelines, several violent visuals were shortened. A close-up of a throat-cutting scene was reduced by 20% (approximately 3 seconds), and a visual involving a woman being slapped saw a similar 20% reduction of 2 seconds. Additionally, the board ordered two-second trimmings for two separate bloodshed shots—specifically, a close-up of a head-hitting sequence and a shot of blood squirting on a woman's face—ensuring the graphic nature of the scenes was slightly toned down for the theatrical release.
The final censor certificate, handed over on February 11, also revealed that O'Romeo is set to be a massive cinematic experience. The film's total runtime is clocked at 178.41 minutes, or 2 hours, 58 minutes, and 41 seconds. This makes it the longest film in the careers of both Shahid Kapoor and Vishal Bhardwaj. The duration was slightly influenced by the CBFC's requirement to lengthen the anti-tobacco and anti-smoking disclaimers and voice-overs, which added nearly 2 minutes of mandatory runtime.
